New Patient Appointment

Your first visit to our office will be a comprehensive examination by the doctor and any necessary x-rays will be taken. At this appointment, we will review dental and medical history, review diagnostic x-rays, and examine your oral health to make a treatment plan specific to your dental needs. After scheduling an appointment, you will be sent a link with new patient paperwork to complete. Please complete this prior to your appointment or arrive 15 minutes early to your appointment.

Please note: We do not start treatment or provide a cleaning on your New Patient Appointment. These appointments will be scheduled after the first patient appointment. We are currently booking cleanings out into October 2025.

Please provide the following information at the time of appointment:

·         If you have x-rays taken within the past 5 years, please have them sent to our office info@dentalwellnessme.com

·         A copy of your dental insurance card

·         Your medication list, if applicable
